Fourteen statements cover the seven areas that shape culture: management commitment, communication, employee involvement, training and information, motivation, compliance with procedures and learning organisation. You rate each one on a five point scale based on what happens day to day, rather than what the policy says.
Your results map to one of the five stages of the HSE Safety Culture Maturity Model, from Emerging through to Continuous Improvement, and show where your strengths and gaps sit across each area. Answer as honestly as you can. A flattering result is not much use to anyone.
